Our Services
Clients who meet with our Case Managers must be diagnosed with a Serious Mental Illness (SMI) and/or substance use disorder and not be an immediate danger to themselves or others. If diagnosed with substance use disorder, the client cannot be currently using substances.
Case Management may be combined with individual and/or group therapy, psychiatric consultation/medication review, nursing services, community living supports (through our Community Support Associates or CSAs), peer support services, assistance with housing or residential placement, as well as home visits (or visits to the client's place of residence).
Services are reviewed on an ongoing basis and are tailored to the specific needs of each person.
Case Managers Often Assist With
- Psychiatric evaluation and review
- Medication management
- Crisis intervention
- Connecting client to Community Support Associates, Occupational Therapy, and Therapeutic Groups
- Linking the client to basic community services (including health care, dental care, food, and housing)
Providing Trauma Informed Care for Breathitt, Knott, Lee, Leslie, Letcher, Owsley, Perry, and Wolfe Counties
